Job summary

AdvisaCare is seeking a compassionate Hospice RN Case Manager to provide one-on-one home hospice care across the Bay City/Midland/Saginaw Tri City area. This full-time role focuses on delivering dignified, at-home end-of-life care with a patient- and family-centered approach.

You will conduct supervisory visits, contribute to interdisciplinary team meetings, and help develop and revise patient/family care plans. A Michigan RN license and hospice experience are required.

Qualifications

• Current Michigan License in good standing.
• Minimum 1 year hospice experience.
• Registered nurse with Hospice experience.
• Empathetic approach to patients and caregivers.

Responsibilities

• Provide one-on-one care in the comfort of home.
• Support patients and families with dignity and heart.
• Conduct supervisory visits for clinical observation of team members.
• Participate in IDG conferences and care plan review.
