Job summary

Absolute Hospice in Green, OH is seeking an experienced Hospice RN Case Manager to join our team. This full-time role focuses on direct patient care, family support, and coordinating an interdisciplinary group to ensure high-quality hospice services.

The RN will manage patient records, educate families on medications and care, and implement the plan of care in collaboration with physicians and care teams.

Qualifications

• The RN provides direct patient care to hospice patients and supports families.
• Manages an interdisciplinary group and supports staff to improve hospice quality.
• Case manages all assigned patient clinical records.

Responsibilities

• Assures the highest quality of care is provided and documented in accordance with the Plan of Care.
• Performs the initial and on-going assessment of the patient.

• Responsible for the education of the patient, family or other caregivers including medication administration, dressing changes, oxygen administration, hospice philosophy and care of the terminally ill patient.

Absolute Hospice is seeking an experienced, compassionate RN to join our hospice team!

The Registered Nurse (RN) will provide direct patient care services to hospice patients and supportive care to the patient's family; manage an interdisciplinary group and support staff to enhance the quality of hospice are, and, case manages all assigned patient clinical records. Some duties include, but not limited to:

• Assures the highest quality of care is provided and documented in accordance with the Plan of Care
• Performs the initial and on-going assessment of the patient
• Helps develop and implement the hospice plan of care as prescribed by the physician and in conjunction with the other members of the group
