Job summary

Absolute Hospice is seeking a compassionate RN to join our hospice team as a Hospice RN Case Manager in Massillon, OH. This full-time role includes a $2,500 sign-on bonus and demands care coordination, patient assessment, and education for patients and families.

The RN will lead an interdisciplinary group, manage patient records, and ensure the highest quality of care in accordance with the Plan of Care, while supporting staff and caregivers in terminally ill care.

Qualifications

• Active RN license in the practicing state.
• Hospice or home health experience preferred.

Responsibilities

• Ensure high-quality care is documented per Plan of Care.
• Perform initial and ongoing patient assessments.

Absolute Hospice is seeking an experienced, compassionate RN to join our hospice team!

The Registered Nurse (RN) will provide direct patient care services to hospice patients and supportive care to the patient's family; manage an interdisciplinary group and support staff to enhance the quality of hospice are, and, case manages all assigned patient clinical records. Some duties include, but not limited to:

• Assures the highest quality of care is provided and documented in accordance with the Plan of Care
• Performs the initial and on-going assessment of the patient
• Helps develop and implement the hospice plan of care as prescribed by the physician and in conjunction with the other members of the group
• Responsible for the education of the patient, family or other caregivers including medication administration, dressing changes, oxygen administration, hospice philosophy and care of the terminally ill patient
