Job summary

BrightSpring Health Services is looking for a passionate Registered Nurse Case Manager for its hospice team in Woodbury, MN. In this full-time position, you will provide expert patient-centered care while ensuring effective pain and symptom management for patients in their homes.

Candidates must be graduates of an accredited nursing school with a current nursing license and at least one year of nursing experience. The ideal candidate is committed to quality care and effective communication with both patients and their families.

Qualifications

• One year nursing experience required, 3+ years preferred.
• Experience in hospice or a similar setting preferred.

Responsibilities

• Assess/monitor physical, emotional, and psychological needs of patients.
• Create hospice care plans that align with the patient’s wishes.
• Provide pain and symptom management.

You Will Do

• Assess/monitor physical, emotional, and psychological needs of patients
• Create hospice care plans that align with the patient’s wishes and goals
• Direct nursing care: administering medications, treatments, and interventions
• Provide pain and symptom management
• Educate and support the patient's family and caregivers
• Collaborate with interdisciplinary team
• Maintain accurate and timely documentation
• Participate in on-call rotation as required by local branch

Qualifications

• Graduate of an accredited nursing school with current licensure to practice in the state of operation
• One year nursing experience required, 3+ years preferred
• Experience in hospice or a similar setting preferred
• Knowledge of the hospice philosophy of care
• Commitment to clinical and documentation excellence
• Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order
