Buckeye Hospice and Palliative Care Services in Athens, Ohio is seeking a dedicated Registered Nurse (RN) Case Manager to deliver compassionate, patient-centered care that respects independence, dignity, and quality of life.

You will perform assessments, develop and adjust care plans, coordinate with families and clinical teams, and participate in 24/7 on-call coverage, supporting patients in home, nursing facilities, and community settings.

Qualifications

• RN license required.
• Valid driver's license and reliable transportation.

Responsibilities

• Provide initial and comprehensive assessments of patient needs, developing personalized care plans to address their health and social needs.
• Implement and revise care plans as needed, ensuring that patients receive the best possible care and services.
• Communicate effectively with patients, families, and healthcare teams to coordinate care delivery and ensure seamless transitions.
• Supervise and instruct direct care staff members and non-professional hospice aide staff to deliver quality care.
• Participate in on-call rotation, providing 24/7 coverage for patients and families in need.
