Job summary

Trinity Health At Home's St. Mary’s Home Care and Hospice in Athens, GA is seeking a Hospice RN Case Manager to provide professional nursing care to hospice clients as prescribed by the physician, in accordance with state laws and organizational policies.

You will manage a caseload of 15–20 patients, collaborate with an interdisciplinary team, and participate in weekly meetings. The position is Full time, mostly Monday–Friday, with a $10,000 signing bonus and Day 1 benefits.

Qualifications

• Graduate of an approved nursing education program.
• Licensure as a Registered Nurse in Georgia.
• One year experience as a professional acute care nurse.
• Home care experience preferred but not required.

Responsibilities

• Provide professional nursing care to hospice clients as prescribed by the physician and in accordance with state regulations and organizational policies.
• Manage and oversee a caseload of hospice patients, collaborating with an interdisciplinary team.
• Participate in weekly interdisciplinary meetings and coordinate with the primary care physician and hospice medical director as needed.
• Occasional weekend coverage when required.
