St. Mary’s Home Care and Hospice in Athens, GA, part of Trinity Health at Home, is seeking a Hospice RN Case Manager to provide professional nursing care to home hospice clients as prescribed by physicians and in line with state nurse practice acts.

You will assess patients, plan care, and supervise outcomes while collaborating with an interdisciplinary team, the primary care physician, and hospice medical director to manage symptoms and support families.

Formación

• Graduate of an approved nursing education program.
• Licensure as a Registered Nurse in the State of Georgia.
• One year experience as a professional acute care nurse.
• Home care experience preferred but not required.

Responsabilidades

• Provide professional nursing care to hospice clients as prescribed by the physician and in compliance with licensure and organizational policies.
• Lead client assessment, care planning, therapeutic intervention, and supervision of care and outcomes.
• Collaborate with interdisciplinary team; attend weekly Interdisciplinary meetings; interact with primary care physician and hospice medical director as needed to manage symptoms.
