Qualifications

• Current RN license in the State of Georgia.
• CPR & First Aid Certification.
• Reliable and legal transportation to and from patient's place of residence.
• Hospice or home care experience is a plus, but kindness and reliability matter the most

Responsibilities

• Serve as clinical anchor for patients and families, ensuring support and informed care at every step.
• Coordinate end-of-life care with the care team and IDT, delivering compassionate service.
• Collaborate with clinicians to provide education and ensure smooth execution of care plans.
