Job summary

Adoration Health is seeking a dedicated Hospice RN Case Manager to join our Independence, OH team. You will provide compassionate, patient-centered care, create personalized hospice plans, and coordinate with an interdisciplinary team to support patients and families.

Qualifications include an accredited nursing degree, current RN licensure, and 1 year of nursing experience with hospice exposure preferred. Valid driver with insured vehicle required.

Qualifications

• Graduate of an accredited nursing school with current licensure to practice in state of operation.
• One year nursing experience required, 3+ years preferred.
• Experience in hospice or a similar setting preferred.
• Knowledge of the hospice philosophy of care.
• Commitment to clinical and documentation excellence.
• Must be a licensed driver with insured vehicle per state requirements.

Responsibilities

• Assess/monitor physical, emotional, and psychological needs of patients.
• Create hospice care plans that align with the patient's wishes and goals.
• Direct nursing care: administering medications, treatments, and interventions.
• Provide pain and symptom management.
• Educate and support the patient's family and caregivers.
• Collaborate with interdisciplinary team.
• Maintain accurate and timely documentation.
• Participate in on-call rotation as required by local branch.
