Job summary

Hospice of North Alabama in Florence, AL is seeking a dedicated RN Case Manager to provide patient‑centered hospice care in the home setting. You will assess needs, develop care plans, administer treatments, and support families, collaborating with an interdisciplinary team to ensure compassionate, evidence‑based care.

This role offers 1:1 patient focus, flexible scheduling, and opportunities for growth within a growing organization.

Qualifications

• Graduate of an accredited nursing school with current licensure to practice in the state of operation.
• One year nursing experience required, 3+ years preferred.
• Experience in hospice or a similar setting preferred.
• Knowledge of the hospice philosophy of care.
• Commitment to clinical and documentation excellence.
• Must be a licensed driver with an automobile that is insured and CPR certification as required.

Responsibilities

• Assess/monitor physical, emotional, and psychological needs of patients
• Create hospice care plans that align with patients' wishes and goals
• Direct nursing care: administering medications, treatments, and interventions
• Provide pain and symptom management
• Educate and support the patient’s family and caregivers
• Collaborate with interdisciplinary team
• Maintain accurate and timely documentation
• Participate in on-call rotation as required by local branch
