Job summary

Hospice of North Alabama is seeking a dedicated RN Case Manager to join our Florence, AL team. This role provides patient-centered hospice nursing, develops care plans aligned with patient goals, administers medications and treatments, and collaborates with an interdisciplinary team to ensure compassionate, comprehensive care in the home setting.

The ideal candidate is a graduate of an accredited nursing program, currently licensed as an RN in Alabama, with at least one year of nursing

Qualifications

• Graduate of an accredited nursing school with current licensure to practice in state of operation.
• One year nursing experience required, 3+ years preferred.
• Experience in hospice or a similar setting preferred.
• Knowledge of the hospice philosophy of care.
• Commitment to clinical and documentation excellence.

Responsibilities

• Assess/ monitor physical, emotional, and psychological needs of patients.
• Create hospice care plans that align with the patients wishes and goals.
• Direct nursing care: administering medications, treatments, and interventions.
• Provide pain and symptom management.
• Educate and support the patient’s family and caregivers.
• Collaborate with interdisciplinary team.
• Maintain accurate and timely documentation.
• Participate in on-call rotation as required by local branch.
