Job summary

Ascend Hospice is hiring an RN Case Manager to manage patient care plans and coordinate nursing services across the care team. The position follows a Monday–Friday schedule with no overnights or weekends, offering flexibility and personalized scheduling.

The ideal candidate will hold an Associate's Degree in Nursing, possess a State RN license, and have hospice experience, along with reliable transportation. Competitive salary and benefits accompany this essential role within our hospice program.

Qualifications

State Certified RN license.
2 years' experience as a professional nurse; hospice experience preferred.
Must have a valid Driver's license and reliable transportation.
Strong verbal and written communication skills.

Responsibilities

Complete initial assessment of patient and family to determine hospice care needs.
Regularly re-evaluate the patient's nursing needs.
Use assessment data to determine care plan needs.
Collaborate with the healthcare team, patient and family to coordinate continuous care.
Counsel patient and family in meeting end-of-life needs.
Initiate appropriate preventive and palliative nursing procedures.
Communicate with the physician regarding patient needs and changes.
Supervise the Hospice Aides.
