Mercy Health Home Care and Hospice by Compassus is seeking a Hospice Registered Nurse / RN Case Manager to provide compassionate, patient‑centered home care. You will manage end‑of‑life care, coordinate with families and the care team, and support patients in maintaining dignity and comfort in their homes.

The role requires an active RN license in the state, two years of RN experience (hospice preferred), a valid driver’s license, CPR/BCLS certification, and a dedication to flexible scheduling

• Two years of RN experience; hospice experience preferred.
• Current driver’s license and reliable vehicle.
• CPR and BCLS certification are required.
