Job summary

Interim HealthCare Hospice & Palliative Care in Ohio is seeking a Hospice Registered Nurse Case Manager (RN) to be called to care when needed, delivering compassionate patient-centered services across Defiance, Henry, Williams, Fulton, Paulding, and nearby counties.

Responsibilities include overseeing care plans with the team, evaluating patient needs, and coordinating with clinicians to adjust care as patient conditions change.

Qualifications

• Bachelor's degree not required but BSN preferred.
• Active RN license in the state of assignment.
• CPR/FA certification is required.
• Reliable transportation to care sites and/or work locations.
• One year of RN experience in hospice or similar setting preferred.

Responsibilities

• Oversee implementation and ongoing assessment of patient care plans with the care team.
• Evaluate patient needs and develop a comprehensive care plan.
• Communicate patient conditions and coordinate with providers as needs evolve.
