Job summary

Purposelink Recruitment is seeking a Registered Nurse Case Manager in Dingmans Ferry, PA. This direct-hire role requires coordinating end‑of‑life care, leading interdisciplinary teams, and ensuring high-quality hospice services.

Expect Monday–Friday work hours (8:00 AM–6:00 PM) with 1 weekend per month and 2 holidays per year, plus local travel across multiple counties. Active PA RN licensure and BLS/ACLS are required.

Qualifications

• High School Diploma or GED required.
• 1+ year Hospice experience.
• 1+ year Management or Leadership experience.
• 1+ year experience in Home Health Care or Epic Home Health or Home Health.

Responsibilities

• Manage patient care within hospice settings and oversee plans of care.
• Coordinate care plans with physicians, social workers, and teams.
• Provide leadership and mentorship to nursing staff.
• Conduct patient assessments and develop individualized care plans.
• Ensure compliance with hospice standards and regulatory requirements.
• Maintain documentation and timely reporting.
• Travel locally to visit patients across counties.
