Qualifications

• 1+ year experience in home health or Epic Home Health.
• BLS/ACLS certifications required.

Responsibilities

• Manage and oversee patient care within hospice settings.
• Conduct comprehensive assessments and develop individualized care plans.
• Coordinate care with physicians, social workers, and interdisciplinary teams.
• Provide leadership, mentorship, and support to nursing staff.
• Ensure compliance with hospice standards, policies, and regulatory requirements.
• Maintain accurate documentation and timely reporting.
• Travel locally to visit patients across assigned counties.

Required Qualifications

• Valid Registered Nurse (RN) license in the state of Pennsylvania (PA)

Certifications (ALL required):

• American Heart Association Basic Life Support (AHA BLS)
• American Heart Association Advanced Cardiac Life Support (AHA ACLS)

Schedule & Shift Details

• Monday–Friday, 8:00 AM–6:00 PM
• On‑Call:
  • One weekend per month
  • Two holidays per year (1 summer, 1 winter)

Travel Requirements

• Local travel required across multiple counties
• Must be comfortable working in field‑based hospice environments
