Job summary

Purposelink Recruitment client is seeking a Registered Nurse Case Manager for hospice in Canonsburg, PA. You will coordinate patient care, support families, and serve as a clinical leader within a compassionate, patient-centered team in a field-based environment.

Responsibilities include creating care plans, collaborating with physicians, social workers and staff, ensuring timely documentation, and guiding nursing colleagues.

Qualifications

• RN license in the state of Pennsylvania (PA) is required.
• 2+ years of Hospice experience.
• 2+ years of Management or leadership experience.
• 1+ years of Home Health/Home Health related experience (Epic Home Health or similar).

Responsibilities

• Coordinate and oversee patient care in hospice settings.
• Develop, implement, and evaluate patient-specific care plans.
• Collaborate with physicians, social workers, and support staff.
• Provide leadership and mentorship to nursing staff.
• Maintain accurate and timely clinical documentation.
• Communicate effectively with patients, families, and caregivers.
• Participate in on-call rotation (evening weekly + weekend monthly).
• Travel locally to patient homes and facilities across multiple counties.
