Job summary

Good Shepherd Hospice is seeking a compassionate Registered Nurse Case Manager to serve patients and families in Marshall and surrounding communities. The RN will assess, plan and provide nursing care to maximize comfort and health, respond to calls, make visits, and handle after-hours admissions and deaths as needed.

The role requires prior hospice/home health/geriatrics experience and a strong commitment to hospice care.

Qualifications

• Registered Nurse currently licensed in the state in which the care is provided.
• Previous experience in hospice, home health, geriatrics or general medical/surgical nursing.
• Demonstrated commitment to the hospice philosophy of care.

Responsibilities

• Receives patient assignments from the Patient Care Manager or Case Manager and coordinates care with physicians and hospice staff.
• Cultivates communication and bereavement skills.
• Maintains regular communication with the attending physician about patient/family care.
• Collects data on physical, psychological, social and spiritual factors influencing patient/family health and updates Plan of Care.
• Maintains up-to-date patient records and ensures Medicare and policy compliance.
• Instructs patient and caregiver on medication administration, safety, personal care, and mobility.
• Attends team meetings and care plan conferences; provides support at time of death.
• Provides 24/7 coverage to patients and families and evaluates aides and LPNs regularly.
