Job summary

Providence Care is hiring full-time Registered Nurses to join our team, delivering Case Management services to patients in homes, assisted living, and long-term care facilities enrolled in our hospice program in the Gainesville area.

Responsibilities include symptom and medication management, collaborating with the IDG and supervising physician, conducting start of care visits, providing education to patients and families, and supervising LPNs and CNAs.

Qualifications

• Must have a RN license unincumbered in the state you are applying.
• Reliable transportation and valid driver's license, clean MVR.
• Experience in Hospice/Home Health is preferred.

Responsibilities

• Provide Case Management to patients and families, including symptom management and medication management, in home health, hospice, and long-term care settings.
• Collaborate with the Interdisciplinary Group (IDG) and supervising physician to oversee patient plans of care.
• Provide supervisory visits and oversight for LPNs and CNAs.
