Ambercare Hospice in Silver City, NM is seeking a compassionate Registered Nurse Case Manager to provide expert clinical care and support to patients and families during life’s meaningful moments.

In this daytime, Monday-Friday role, you will coordinate patient visits, assess needs, collaborate with the interdisciplinary team, and ensure timely care plans while maintaining thorough documentation.

Qualifications

• Graduate from an accredited registered nursing program.
• Licensed as a registered nurse in the state of practice.
• Must possess current CPR, First Aid Certification.
• Valid driver's license and proof of insurance is required.
• Knowledgeable of quality assessment and assurance procedures.

Responsabilités

• Assume primary responsibility for patient and family caseload and manage admission processing, planning, implementation, and evaluation.
• Assess data from patient, family, and interdisciplinary team to determine needs.
• Coordinate with attending physician and agencies for optimal care.
• Provide regular visits to reassess needs and update the plan of care.
• Maintain accurate records and supervise Home Health Aide delivery as required.
