Job summary

Ambercare Hospice in Silver City, NM is seeking a compassionate Registered Nurse Case Manager to provide expert clinical care to patients and families, coordinating visits and delivering high-quality hospice services. You will lead the nursing process from assessment to evaluation, work with an interdisciplinary team, and ensure timely documentation, with a focus on comfort, dignity, and quality of life.

Qualifications

• Graduate from an accredited registered nursing program.
• Licensed as a registered nurse in the state of practice.
• Current CPR and First Aid Certification.
• Valid driver’s license and proof of insurance is required.
• Knowledgeable of quality assessment and assurance procedures.

Responsibilities

• Assume primary responsibility for patient and family caseload, including assessment, eligibility, admission processing, planning, implementation and evaluation phases of the nursing process.
• Obtain data from patient’s physical, psychological, social, and spiritual factors and initiate interventions by the interdisciplinary team.
• Initiate communication with attending physician and interdisciplinary team to coordinate optimal care.
• Provide regular visits to reassess patient needs and update the plan of care.
• Perform clinical pronouncement of death and provide support to the family.
• Maintain accurate patient records, charts, and care plans timely.
• Supervise and document delivery of care by Home Health Aide as required by guidelines.
