Job summary

Ambercare Hospice in Silver City, NM is seeking a compassionate Registered Nurse Case Manager to provide expert clinical care to patients and families, coordinating visits and managing the nursing process to ensure comfort and quality of life. Qualifications

• Graduate from an accredited registered nursing program.
• Licensed as a registered nurse in the state of practice.
• Must possess current CPR, First Aid Certification.
• Must be knowledgeable of quality assessment and assurance procedures.
• Valid driver’s license and proof of insurance is required.

Responsibilities

• Assume primary responsibility for patient and family caseload, including assessment, eligibility, admission processing, planning, implementation and evaluation phases of the nursing process.
• Obtain data from patient and family factors that may impact care and coordinate with interdisciplinary team.
• Initiate communication with attending physician and other agencies to coordinate optimal care.
• Provide regular visits to reassess patient needs and update the plan of care.
• Maintain accurate patient records and care plans timely.
• Supervise and document delivery of care by Home Health Aide as required.
