Job summary

Amedisys is seeking an RN Case Manager for Hospice in Mystic, CT. This full-time, dayshift role covers Mystic and surrounding areas, coordinating patient care at home and ensuring seamless service delivery.

Qualifications

• One year of nursing experience.
• Current CPR certification.
• Valid driver's license and reliable transportation.

Responsibilities

• Performs patient assessments and collaborates with the care team to develop and implement a plan of care.
• Makes referrals to other hospice care disciplines as needed or documents rationale.
• Maintains individualized patient care progress records with accuracy and timeliness.
• Records pain/symptom management changes and outcomes.
• Educates patients, families, and caregivers in care techniques.
• Communicates with clinical manager and care team regularly.
• Performs on-call responsibilities and other duties as assigned.
