KLA Industries is seeking a Hospice RN Case Manager to coordinate and oversee patient care in the Dallas area south of Downtown Dallas. This is a full-time role requiring clinical assessment, symptom management, and collaboration with the interdisciplinary team to support patients and families.

The RN will work with the interdisciplinary team to develop individualized plans of care, monitor symptoms, and communicate progress to leadership.

Qualifikationen

• Current Registered Nurse license valid for practice in Missouri
• Current driver's license
• Current CPR certification
• Two years of Registered Nurse experience in a clinical care setting
• One year of full-time hospice experience preferred
• Reliable transportation and valid automobile liability insurance
• Ability to work independently and make sound clinical decisions
• Strong written, verbal, organizational, and interpersonal skills

Outstanding hospice provider seeks a full-time RN Case Manager. This nurse will coordinate and oversee patient care, develop and implement individualized plans of care, provide clinical assessment and symptom management, and work closely with the interdisciplinary team to support patients and families.
