Job summary

HIA Hospice in Detroit Lakes, MN seeks a dedicated Hospice Staff RN/Case Manager to provide nursing care for hospice patients in coordination with the interdisciplinary team. You will support admissions, assessments, symptom management, and emotional support in alignment with the agency's mission.

Qualifications

• Minimum of RN degree; BSN preferred.
• Minimum of one year as hospital staff nurse and/or two years in health care.
• Strong interpersonal communication, counseling, and teaching skills.
• Flexibility in scheduling; team player.
• Familiarity with dying and death concepts strongly preferred.
• Current state RN licensure in Minnesota and North Dakota.

Responsibilities

• Provide nursing care to hospice patients in coordination with the IDG.
• Include hospice referrals, patient admissions, physical assessments, symptom management and emotional support.
• Adhere to patient goals and agency policies in collaboration with primary physicians and medical director.
