Job summary

Day City Hospice in Dayton, OH is seeking a compassionate RN Case Manager to guide patients and families through the hospice journey. You will serve as the primary clinical resource, coordinating care with physicians and the interdisciplinary team.

Qualifications

• Graduate of an accredited Registered Nursing program.
• Current Registered Nurse (RN) license in the State of Ohio.
• Current CPR and First Aid certification.
• Knowledge of quality assessment and performance improvement practices.
• Valid driver's license, reliable transportation, and proof of auto insurance.

Responsibilities

• Manage and coordinate patient and family caseloads throughout the hospice care continuum
• Conduct comprehensive assessments and determine patient care needs
• Develop, implement, and update individualized care plans
• Collaborate with physicians and interdisciplinary team members to ensure exceptional patient outcomes
• Identify physical, emotional, social, and spiritual needs and facilitate appropriate interventions
• Perform regular patient visits and reassess care needs as conditions change
• Provide education, support, and counseling to patients and loved ones
• Coordinate services with community partners and healthcare providers
• Complete accurate documentation and maintain compliance with regulatory requirements
• Supervise and support Home Health Aides in delivering quality patient care
• Provide end-of-life support and clinical pronouncement of death while offering compassionate family guidance
