Day City Hospice in Dayton, OH is seeking an experienced Registered Nurse Case Manager to provide leadership in clinical care and support to patients and families.

You will work within a compassionate team to coordinate assessments, care plans, and interventions, ensuring high-quality end-of-life care with dignity and respect. Day City Hospice offers a supportive culture and comprehensive benefits.

Qualifications

• Graduate of an accredited Registered Nursing program.
• Current RN license in the State of Ohio.
• Current CPR and First Aid certification.
• Valid driver's license and proof of auto insurance.

Responsibilities

• Manage and coordinate patient and family caseloads throughout the hospice care continuum.
• Conduct comprehensive assessments and determine patient care needs.
• Develop, implement, and update individualized care plans.
• Collaborate with physicians and interdisciplinary team to ensure optimal patient outcomes.
• Identify needs (physical, emotional, social, spiritual) and coordinate interventions.
• Perform regular patient visits and reassess care needs.
• Provide education, support, and counseling to patients and families.
• Coordinate services with community partners and healthcare providers.
• Complete accurate documentation and maintain regulatory compliance.
• Supervise and support Home Health Aides in delivering quality care.
