Job summary

Corewell Health is seeking a Hospice Nurse Case Manager to coordinate end-of-life care, assess patient needs, and develop individualized care plans in collaboration with an interdisciplinary team.

The role involves symptom management, patient and family education, and travel to patients’ homes or facilities to deliver direct hospice care. Michigan RN licensure and driver’s license are required; a Bachelor’s degree is preferred.

Qualifications

• Associate’s degree in nursing is required.
• 2 years of relevant experience is required.
• Michigan RN license upon hire is required.
• State of Michigan Driver’s license upon hire is required.
• Bachelor’s degree is preferred.

Responsibilities

• Conduct comprehensive patient assessments and develop individualized care plans.
• Administer medications and manage symptoms for hospice care.
• Collaborate with an interdisciplinary team for holistic care.
• Educate patients and families about disease progression and care options.
• Provide emotional and psychosocial support to patients and families.
• Maintain accurate documentation and ensure compliance with hospice regulations.
• Travel to patients' homes, nursing facilities, and hospitals for direct care.
