Job summary
Brookhaven Hospice in Westborough, MA is seeking a dedicated Registered Nurse Case Manager (RNCM) to join our compassionate team. This full-time role delivers hands-on hospice nursing, coordinates services, and guides patients and families through their hospice journey.
The RNCM will collaborate with our interdisciplinary team, provide comfort-focused care, educate families, and participate in call coverage.
Qualifications
- Current unencumbered RN license.
- Minimum 1 year RN experience in medical-surgical/acute care; hospice experience preferred.
- Proficient with computers and EMR systems; strong typing skills.
- CPR certification as required by state; valid driver license and reliable transportation.
Responsibilities
- Respond promptly to clinical referrals with compassionate, expert care.
- Collaborate with Clinical Director to assess eligibility for hospice services.
- Provide direct nursing care focused on comfort and quality of life.
- Deliver nursing care rooted in established principles to ease pain and provide solace.
- Educate patients and families to participate in personalized care plans.
- Share call duties and be available for critical situations.
