Job summary

Brookhaven Hospice is seeking a Full Time Hospice Registered Nurse Case Manager to join our nursing team providing exceptional end-of-life care throughout the community.

In this role you will deliver hands-on hospice nursing care, coordinate services, and guide patients and families through every step of their journey. The position requires an RN license and at least 1 year of RN experience in med-surg/acute care, with EMR proficiency and CPR certification where required.

Qualifications

• Hold current unencumbered RN license.
• Minimum one (1) year RN experience in medical-surgical/acute care; hospice experience a plus.
• Must be computer proficient in typing and various programs, including EMR background.
• Current CPR certification if required by state.

Responsibilities

• Empower Through Engagement: Respond to clinical referral information with care and expertise, addressing inquiries and requests with compassion.
• Collaborative Care Champion: Partner with Clinical Director to assess eligibility and suitability for hospice services.
• Nurturing Direct Care: As an RN Case Manager, deliver hands-on nursing care and support patient comfort.
• Nursing with Heart: Provide nursing care rooted in proven principles to ease pain and bring solace.
• Educator and Advocate: Educate patients and families to create personalized care plans.
• Call of Compassion: Share call duties as part of the care team.
