Job summary

Anchor Hospice and Palliative Care in San Joaquin County seeks an RN Case Manager to administer nursing care per the plan of care for terminally ill patients. You will collaborate with physicians, social workers, and other care team members to coordinate comprehensive nursing assessments and palliative interventions for families.

Requirements include California RN license in good standing, CPR certification, and a valid driver’s license with reliable transportation.

Qualifications

• Must be a licensed RN in California.
• Hospice experience preferred.
• CPR certification required.
• Valid driver’s license and automobile insurance.
• Reliable transportation.

Responsibilities

• Organize patient care and develop the interdisciplinary plan of care with Physicians, Medical Directors and team members.
• Provide comprehensive nursing assessments and implement palliative interventions for families.
• Assess and manage pain and symptoms to enhance quality of life.
• Collaborate with the interdisciplinary team to deliver high-quality hospice care.
