Job summary

Anchor Hospice and Palliative Care in California seeks an experienced RN Case Manager to administer nursing care according to the plan of care for terminally ill patients in San Joaquin County.

You will work as part of an interdisciplinary team with physicians, medical directors, social workers and chaplains to organize patient care, teach pallia­tive interventions, manage pain, and help families through life-limiting transitions.

Qualifications

• Licensed Registered Nurse in California in good standing.
• Hospice experience preferred.
• Excellent interpersonal and family communication skills.
• Current CPR certification.
• Valid driver's license and automobile insurance.
• Reliable transportation.

Responsibilities

• Collaborate with Physicians, Medical Directors, Social Worker, Chaplain and CNAs as part of the interdisciplinary team.
• Organize patient care and develop the interdisciplinary plan of care.
• Provide comprehensive nursing assessments and education on pallia­tive interventions to families.
• Assess and implement a pain/symptom management program to improve quality of life.
• Deliver the highest level of hospice care to patients and families during transitions.

We are currently seeking an RN (Registered Nurse) Case Manager to administer nursing care according to the plan of care to terminally ill patients. As a privileged member of Anchor Hospice and Palliative Care, you will provide dignified and compassionate care to patients and families experiencing a life-limiting illness during their time of transitions. Requirements:

• Licensed Registered Nurse, in good standing in the State of California required.
• Hospice experience preferred
• Must possess excellent interpersonal skills particularly with family cultural/sensitivity
• Current CPR certification
• Valid driver’s license and current automobile insurance
• Reliable transportation

Duties and Responsibilities:

Case Managers work collaboratively with Physicians, Medical Directors, Patient Care Coordinator, Social Worker, Chaplain and Certified Nursing Assistants as an integral member of the interdisciplinary team. Case Managers assume the overall responsibility for organizing patient care, develop the interdisciplinary plan of care, provide comprehensive nursing assessments, teach appropriate palliative interventions to family members, assess and implement a pain/symptom management program to enhance quality of life and provide the highest level of hospice care to our patients.
