Job summary

Caring Hospice Services seeks a Registered Nurse Case Manager in Pennsylvania to perform ongoing patient assessments and keep thorough notes on each care plan. You will stay in close contact with patient families and caregivers to ensure comfort and effective symptom control.

The role requires an RN license, at least an associate/bachelor's/master's degree in nursing, and the ability to work independently in a hospice setting.

Qualifications

• Must hold a Nursing degree and PA RN license.
• Demonstrated integrity and sound judgment as a patient advocate.
• Ability to work independently and with diverse teams.
• Experience in Med-Surg, Home Care, or Hospice preferred.

Responsibilities

• Provide nursing care with emphasis on pain and symptom management.
• Supervise Home Health Aides and coordinate patient care.
• Refer families to community resources and maintain confidentiality.
• Deliver care directives to families and caregivers.
