Qualifications

• Current Maryland State license as a Registered Nurse is required.
• Two years of nursing experience, or equivalent combination of education and experience.

Responsibilities

• Completes initial assessment of patient and family to ascertain home care needs.
• Prepares and implements the care plan for the patient/family unit.
• Provides direct nursing care to patients in collaboration with the primary physician.
• Plans and supervises delivery of care by the HHA.
• Updates primary physician and interdisciplinary team as needed.
