Concho Hearts Hospice in Justin, TX is seeking a dedicated Hospice RN Case Manager to join our full-time team. You will manage a caseload of hospice patients in the Justin area and provide skilled nursing care in patients’ homes and facilities.

We value compassionate, patient-centered care and collaboration with physicians and the interdisciplinary team. You will develop and update individualized plans of care, complete visits, admissions, recertifications, and documentation.

Qualifications

• Active Texas RN license in good standing.
• Strong assessment and critical-thinking abilities.
• Excellent communication and teamwork in field care.
• Ability to work independently in patients’ homes.
• Willingness to travel locally with reliable transportation.

Responsibilities

• Manage a caseload of hospice patients in the Justin area.
• Provide skilled nursing care in patients’ homes and facilities.
• Complete routine visits, admissions, and recertifications.
• Develop and update individualized plans of care.
• Collaborate with physicians and the interdisciplinary hospice team.
• Provide education and emotional support to patients and families.
• Maintain accurate documentation using a company-provided tablet.
