You will manage a local caseload, provide skilled nursing care in patients' homes and facilities, and collaborate with physicians and the interdisciplinary team to update care plans.

Qualifications

• Active Texas RN license.
• Strong assessment and critical thinking.
• Excellent communication and teamwork.
• Ability to work independently in the field.
• Reliable transportation and willingness to travel locally.

Responsibilities

• Manage a caseload of hospice patients in the Fort Worth area.
• Provide skilled nursing care in patients' homes and facilities.
• Complete routine visits, admissions, and recertifications.
• Develop and update individualized plans of care.
• Collaborate with physicians and the interdisciplinary hospice team.
• Provide education and emotional support to patients and families.
• Maintain accurate documentation using a company-provided tablet.
