Job summary

Vitality Hospice & Palliative Care in New Jersey is seeking a Registered Nurse Case Manager (RNCM) to join a compassionate interdisciplinary team. This full-time role requires strong clinical skills, effective communication, and the ability to travel to patient residences.

You’ll develop and coordinate individualized care plans, collaborate with hospice physicians and the IDT, educate families on disease progression and symptom management, and ensure timely documentation in line with hospice

Qualifications

• Current RN license valid in New Jersey.
• Hospice or home health experience preferred.
• Strong communication, organizational and interpersonal skills.
• Valid driver’s license with reliable vehicle and current auto insurance.
• Willingness to travel within the service area.

Responsibilities

• Conduct admissions and ongoing nursing assessments.
• Develop and implement individualized plans of care based on each patient's needs and goals.
• Collaborate closely with hospice physicians and the interdisciplinary team (IDT).
• Supervise and coordinate care with home health aides, social workers, and chaplains.
• Educate patients and caregivers about disease progression, symptom management, and resources.
• Ensure timely and accurate documentation in compliance with hospice standards.
• Promote a culture of excellence, empathy, and accountability in every interaction.

Salary: $85,000–$100,000 per year
