The Employee Connect is seeking a Hospice Registered Nurse Case Manager in Laramie, WY. This full-time role supports patients in home and facility settings with a compassionate, interdisciplinary team approach.

You will conduct admissions, visits, and care planning while delivering expert symptom management and education to families. Ideal candidates hold an active RN license, CPR/BLS, and at least one year of hospice or palliative care experience.

Qualificações

• Active, unrestricted RN license in state of practice or eligible compact license.
• Current BLS or CPR certification.
• One year or more of RN experience; hospice, home health, oncology, or palliative care preferred.
• Valid driver license with reliable transportation and current auto insurance.

Responsabilidades

• Manage a caseload of hospice patients across home and facility settings.
• Complete admission, routine, and recertification visits and assessments.
• Develop and update individualized plans of care with the interdisciplinary team.
• Provide pain and symptom management in collaboration with the hospice physician.
• Educate patients and families about the dying process, medications, and comfort measures.
• Coordinate care with aides, social workers, chaplains, and volunteers.
• Document visits accurately in the electronic medical record.
• Participate in weekly interdisciplinary group meetings.
• Take part in on-call rotation and respond to urgent needs.
• Support families with dignity and compassion through end-of-life care.
