Job summary

Trinity Hospice is seeking an experienced RN Case Manager in the Houston area to join a compassionate interdisciplinary team. The role focuses on comprehensive assessments, individualized care plans, and coordinated care across physicians, social workers, chaplains, and hospice aides.

You will manage pain and symptom control, document thoroughly, and participate in IDT meetings to ensure high-quality patient care. A valid RN license and driving ability are required.

Qualifications

• Current RN license in the state you will work.
• 1 year RN Case Manager or hospice experience preferred.
• Valid driver’s license and reliable transportation.
• Strong clinical judgment and communication skills.

Responsibilities

• Perform comprehensive nursing assessments for patients and families.
• Develop, implement, and evaluate patient-centered care plans.
• Provide skilled nursing interventions including pain management and symptom control.
• Coordinate care as the primary RN case manager across the IDT.
• Collaborate with physicians, social workers, chaplains, and aides.
• Maintain thorough nursing documentation per regulatory standards.
• Participate in IDT meetings and care conferences.
