Job summary

Trinity Hospice in Tulsa, OK is looking for an experienced RN Case Manager to provide comprehensive nursing assessments, develop and manage patient-centered care plans, and coordinate care across an interdisciplinary team. You will manage pain and symptom control, medication administration, wound care, and ongoing monitoring to support comfort and quality of life.

Qualifications

• Current Registered Nurse license in the state where you will be working.
• One year of experience as an RN Case Manager or similar role in a hospice setting preferred.
• Must have a valid driver’s license and dependable transportation.

Responsibilities

• Perform comprehensive nursing assessments of patients and families to identify physical, psychosocial, and emotional needs.
• Develop, implement, and evaluate individualized, patient-centered care plans focused on comfort and quality of life.
• Provide skilled nursing interventions including pain and symptom management, medication administration, wound care, and ongoing clinical monitoring.
• Recognize changes in condition using clinical judgment and intervene appropriately.
• Serve as the primary RN case manager, coordinating care across the interdisciplinary team.
• Collaborate with physicians, nurse practitioners, social workers, chaplains, and hospice aides to ensure seamless care delivery.
• Facilitate communication among team members to support timely care planning.
• Maintain accurate nursing documentation in accordance with regulatory standards (Wellsky/Consolo).
• Participate in interdisciplinary team meetings and care conferences.
