Job summary

Hearth Hospice is seeking a dedicated Registered Nurse Case Manager to provide hands‑on hospice care throughout the Savannah area. You’ll coordinate services, support patients and families, and partner with our interdisciplinary team to ensure compassionate, clinical excellence in end‑of‑life care.

The role follows a Monday–Friday schedule (8:00 am–4:30 pm) serving Chatham, Effingham, Bryan and Liberty counties, with comprehensive benefits and growth opportunities within Hearth Hospice.

Qualifications

• Hold current unencumbered RN license and be eligible to practice as an RN.
• Minimum 1 year RN experience in medical‑surgical/acute care; hospice experience a plus.
• Proficient computer skills including EMR; CPR certification if state requires.
• Valid driver's license and reliable transportation.

Responsibilities

• Empower patients and families through compassionate engagement and education.
• Collaborate with Clinical Director to evaluate eligibility and coordinate care.
• Deliver direct hospice nursing care and ensure patient comfort and quality of life.
• Apply nursing skills to manage symptoms and provide relief in line with best practices.
• Educate families to participate in personalized care plans.
• Share call duties and respond to urgent clinical referrals with care.
