Job summary
Hospice of the West is seeking a Full Time Hospice Registered Nurse Case Manager (RNCM) to join our nursing team delivering compassionate end‑of‑life care in the East Valley. You will provide hands‑on hospice nursing, coordinate services, and guide patients and families through the journey with clinical excellence and connection.
Schedule: 8am‑5pm with on‑call shifts evenings and weekends as needed. We offer a supportive, growth‑oriented environment and a comprehensive benefits package including
Qualifications
- Hold current unencumbered license as a Registered Nurse.
- Minimum one year experience as an RN in a medical surgical/acute care setting.
- CPR certification preferred/required by state.
- Must be computer proficient with EMR systems.
Responsibilities
- Empower Through Engagement: respond to clinical referrals with care and expertise.
- Collaborative Care Champion: assess eligibility for hospice services with Clinical Director.
- Nurturing Direct Care: coordinate care while ensuring comfort and quality of life.
- Nursing with Heart: deliver top‑notch care rooted in proven nursing principles.
- Educator and Advocate: educate patients and families to create personalized care plans.
- Call of Compassion: participate in on-call duties when needed.
