Job summary

Hospice of the West is seeking a Full Time Hospice Registered Nurse Case Manager (RNCM) to join our compassionate team delivering end-of-life care across the East Valley. You will provide hands-on nursing, coordinate services, and guide patients and families through their hospice journey.

The schedule is 8am-5pm with on-call shifts evenings and weekends as needed; we support professional growth and a patient-centered mission to bring dignity, peace, and connection to every life we touch.

Qualifications

• Hold current unencumbered license as a Registered Nurse.
• Minimum one (1) year experience as an RN in a medical surgical/acute care setting. Hospice exp a plus!
• Must be computer proficient with typing and EMR background.
• Possess and maintains current CPR certification if required by state.
• Current driver's license, vehicle insurance, and access to dependable transportation.

• Empower Through Engagement: Respond to clinical referral information with care and expertise, addressing inquiries and requests with a compassionate touch.
• Collaborative Care Champion: Partner with Clinical Director to assess eligibility and suitability for hospice services.
• Nurturing Direct Care: As an RN Case Manager, coordinate care, ensuring patient comfort and quality of life.
• Nursing with Heart: Deliver top-notch care rooted in proven nursing principles.
• Educator and Advocate: Educate patients and families to participate in personalized care plans.
• Call of Compassion: Share in call duties to be there when it matters most.

Are you a compassionate Registered Nurse (RN) who finds purpose in supporting patients and families through meaningful moments of care? Hospice of the West is seeking a Full Time Hospice RN Case Manager (RNCM) to join our dedicated nursing team providing exceptional end-of-life care throughout the community.

In this rewarding role, you’ll deliver hands-on hospice nursing care, coordinate services, and guide patients and families through every step of their hospice journey. As a valued member of our interdisciplinary care team, you’ll help ensure every patient receives The Best Care Possible — care grounded in compassion, clinical excellence, and connection.

$83,000 - $94,000 per year (Average Pay Range). The pay range listed represents a general posting guideline for the role and is not a fixed offer. Final compensation will be determined based on the candidate’s relevant experience, qualifications, and the specific responsibilities of the position. The exact compensation rate will be discussed and confirmed at the conclusion of the interview process.
