Job summary
Hearth Hospice is seeking a Full Time Hospice RN Case Manager (RNCM) to join our dedicated nursing team. You will deliver hands-on hospice nursing care, coordinate services, and guide patients and families through every step of the hospice journey.

Qualifications
- Hold current unencumbered Registered Nurse license.
- Minimum one year experience as an RN in medical-surgical/acute care; hospice experience a plus.
- Must be computer proficient in typing and various programs, including EMR.
- Current CPR certification if required by state.
- Valid driver's license and reliable transportation.
Responsibilities
- Deliver hands-on hospice nursing care.
- Coordinate services and guide patients and families through the hospice journey.
- Collaborate with interdisciplinary care team to ensure high-quality care.
- Provide nursing care rooted in compassion and clinical excellence.
- Educate and empower patients and families to participate in care plans.
