Qualifications

• Licensed Registered Nurse in Texas.
• Valid driver’s license and reliable transportation.
• At least 1 year of nursing experience.
• Hospice case management experience preferred (2+ years).
• Proficient with computers; Microsoft Office experience preferred.

Responsibilities

• Meet with patients and families to explain scope of hospice services; coordinate admission and ongoing plan of care.
• Provide nursing care using the nursing process; assess and re-evaluate patient status.
• Develop care plans with goals based on nursing diagnosis and patient/family input.
• Administer medications and treatments as prescribed by physician.
• Coordinate patient care as Case Manager under supervision.
• Document and communicate patient condition, changes, and orders to physicians.
