Job summary

Hospice of the West is seeking a compassionate Registered Nurse Case Manager (RNCM) to join our dedicated nursing team in Surprise, AZ. This full-time role covers the Surprise, Sun City West, and Sun City Grand territory, with hours 8 am–5 pm Monday through Friday.

You will provide hands-on hospice nursing, coordinate services, and support patients and families through their hospice journey. The position emphasizes compassionate care grounded in clinical excellence and connection, with a

Qualifications

Hold current unencumbered Registered Nurse license.
Minimum one (1) year RN experience in medical surgical/acute care setting; hospice a plus.
Computer proficiency including EMR systems; good typing skills.
Current CPR certification if required by state.
Valid driver's license, vehicle insurance, and reliable transportation.

Responsibilities

Respond to clinical referral information with care and expertise.
Collaborate with Clinical Director to assess eligibility for hospice services.
Provide direct patient care and coordinate services to maintain comfort and quality of life.
Deliver nursing care rooted in proven principles to ease pain and provide solace.
Educate and advocate for patients and families to participate in care plans.
Participate in on-call duties as part of the team.
