Hospice of the West in Tucson is seeking a Full-Time Hospice Registered Nurse Case Manager to join our compassionate team. You will deliver hands-on hospice nursing, coordinate services, and guide patients and families through the care journey.

Qualifications

• Hold current unencumbered RN license.
• Minimum 1 year RN experience in medical-surgical/acute care; hospice experience a plus.
• Proficient in typing and EMR software.
• CPR certification current if required by state.
• Valid driver's license and reliable transportation.

Responsabilités

• Empower Through Engagement: address referrals with care and expertise.
• Collaborative Care Champion: assist Clinical Director in evaluating client eligibility.
• Nurturing Direct Care: provide hands-on hospice nursing and comfort.
• Nursing with Heart: deliver care rooted in evidence-based practices.
• Educator and Advocate: educate patients/families for personalized care plans.
• Call of Compassion: participate in on-call duties as needed.
