Job summary

Bristol Hospice in McAllen, TX is seeking a Registered Nurse Case Manager for day shift to provide compassionate, whole-person hospice care and advocate for patients and families. This role emphasizes collaboration with interdisciplinary teams, accurate documentation, and ongoing patient support in a mission-driven environment.

The RN must hold a valid Texas license, CPR certification, and possess strong observation and communication skills.

• Strong observational and communication skills—both written and verbal
• A flexible, team-oriented attitude and willingness to support additional duties as needed

Responsibilities

• Develop and implement individualized care plans based on current and prior medical history.
• Deliver attentive, one-on-one support to patients and their families, ensuring comfort, dignity, and understanding
• Conduct thorough assessments of physical, emotional, and spiritual needs throughout the care journey
• Accurately monitor and document vital signs including temperature, pulse, respiration, and blood pressure
• Administer physician-ordered medications and treatments with skill and empathy
• Educate patients and their families on care techniques and symptom management
• Track and communicate changes in patient condition, ensuring timely and effective responses
• Keep detailed and up-to-date clinical notes to reflect patient progress and quality of care provided
• Collaborate with interdisciplinary teams to ensure comprehensive care delivery
• Contribute to other supportive duties as part of a compassionate, mission-driven team
• And additional duties as assigned
