Job summary

Bristol Hospice - High Desert in Klamath Falls, OR seeks a Registered Nurse Case Manager to provide compassionate, patient- and family-centered end-of-life care. You will build individualized care plans and work closely with families to ensure comfort and dignity throughout the care journey.

This full-time, hourly position offers daytime shifts Monday through Friday, a supportive team, and comprehensive benefits.

Qualifications

• Valid RN license in good standing in the state you will work in.
• Valid driver’s license and reliable transportation.
• CPR certification (current).
• Strong observational and communication skills, both written and verbal.
• Flexible, team-oriented attitude and willingness to support additional duties as needed.

Responsibilities

• Develop and implement individualized care plans based on current and prior medical history.
• Deliver attentive, one-on-one support to patients and their families, ensuring comfort, dignity, and understanding.
• Conduct thorough assessments of physical, emotional, and spiritual needs throughout the care journey.
• Accurately monitor and document vital signs including temperature, pulse, respiration, and blood pressure.
• Administer physician-ordered medications and treatments with skill and empathy.
• Educate patients and their families on care techniques and symptom management.
• Track and communicate changes in patient condition, ensuring timely and effective responses.
• Keep detailed and up-to-date clinical notes to reflect patient progress and quality of care provided.
• Collaborate with interdisciplinary teams to ensure comprehensive care delivery.
