Job summary

Bristol Hospice in Grass Valley, CA is seeking an RN Case Manager to provide compassionate, whole-person hospice care and advocate for patients and families. This role focuses on developing individualized care plans, delivering one-on-one support, and guiding families through the care journey with dignity.

You will collaborate with interdisciplinary teams, monitor vital signs, administer medications, and educate families on symptom management.

Qualifications

• A valid Registered Nurse license in good standing in the state you will work in.
• CPR certification (current)
• Strong observational and communication skills—both written and verbal
• A flexible, team-oriented attitude and willingness to support additional duties as needed

Responsibilities

• Develop and implement individualized care plans based on current and prior medical history.
• Deliver attentive, one-on-one support to patients and their families, ensuring comfort, dignity, and understanding.
• Conduct thorough assessments of physical, emotional, and spiritual needs throughout the care journey.
• Accurately monitor and document vital signs including temperature, pulse, respiration, and blood pressure.
• Administer physician-ordered medications and treatments with skill and empathy.
• Educate patients and their families on care techniques and symptom management.
• Track and communicate changes in patient condition, ensuring timely and effective responses.
• Keep detailed and up-to-date clinical notes to reflect patient progress and quality of care provided.
• Collaborate with interdisciplinary teams to ensure comprehensive care delivery.
